Lessons learned: Don,t be distracted in the wheelhouse. Make all required safety notifications on appropriate channels. Be ALERT!!

Besides the obvious neglect of the tugboat operator, what I'd like to know is why the duckboat threw out an anchor dead center of the river channel. From my understanding they forgot to put the radiator cap on, Captain mistook the steam for fire and dropped anchor dead center of the channel. I try to put myself in the same situation and my first instinct would be to grab the extinguisher and put out the "fire". I guess with the heavy current in the river maybe the protocol is different, but if I'm dropping anchor in a channel like that I am going to be all over the intercom repeating to CG that I am an anchored hazard in the middle of a channel, not standing on the hood of my boat.

Here is the report from the National Transportation Safety Board. It is quite lengthy but well worth reading.
