Here is a great guide to follow in case you have to evacuate during a Hurricane.  FEMA has some great informational sheets as well.  This one pertains to just Hurricanes before, during and after.  It is a good read for getting things together.

I'd like to emphasize that if there is rising water nearby, leave earlier then later.  Most people underestimate the rate at which flood waters rise.
We had extreme flooding at work last year.  Most of those we had to rescue needed rescue becasue they waited till it was too late.  This makes it difficult and dangerous for the victims and rescuers. 
We had rescues where we took people out of 2nd floor windows and off roofs as the only option.  You should have the common sense to leave before it gets anywhere near that bad.
